The patients
included in these tables are all ESRD beneficiaries who were
Medicare Entitled at the time of ESRD Initiation for
Incident Cohort and at December 31 for Point Prevalent
Cohort. In other words, these patients could be Medicare
Primary, MSP (Medicare as Secondary Payor), or MCO (Managed
Care Organizations).
